Passport Basics:

  • REQUIRED!! A check, cashier's check or money order for application fee (See Passport Fees section) payable to the 'US Dept of State'.
  • DO NOT sign the passport application until requested to do so.
  • An adult passport (16 years of age and older) is valid for 10 years.
  • A child's passport (ages 0-15 years old) is valid for 5 years.
  • If a name is different than the name on the birth certificate, you may be required to provide a legal document (such as a certified copy of a marriage license) to verify the name change.


Finding the Right Form

Applying for a U.S. Passport can take a lot of time at the Passport Office. Save yourself some time! Find the form or forms you need online first. Fill them out, print them out, and bring them with you to the Passport Office.

Proof of Citizenship
  • If born within the United States you must provide a certified copy of your birth certificate. (DOCUMENTS NOT ACCEPTABLE include: hospital certificate, baptism, notification-of-birth and California abstracts of birth.)
  • If born outside of the United States you must provide a Certificate of Citizenship/Naturalization or report of birth abroad.
  • For a complete list of all forms of acceptable citizenship documentation visit the U.S. Dept of State's website.
Personally Appear
  • ALL APPLICANTS (including children) must personally appear when applying for a passport.
  • Children under 16 years of age must have both parents present.
    • IF one of the child's parents can not personally appear, download and complete Form DS- 3053 Statement of Consent for Issuing a Passport to a Child.
    • IF one parent can not be located or contacted to personally appear, download and complete Form DS-5525 Statement of Special Family Circumstances.
  • Children 16 and 17 years of age must have one parent present.
  • Social security numbers must be provided for all passport applicants!
Passport Renewals

If you have had a passport within the last 15 years and were at least 16 years of age or older at the time of issuance, you may obtain a form and submit you own application.

Passport Photo Requirements

  • One U.S. passport photograph is required with the application.
  • Passport photographs must have been taken in the last 6 months.
  • Pictures must be in color and on a white or light colored background.
  • Pictures must be two-inches square. (Image size of the head must be between 1 inch and 1 3/8 inches including hair.) See a qualified passport photographer.

Passport photographs are available in the Clerk’s Office. Cost is $10.00 per person.

Identification Requirements

Common primary identity documents that may be presented alone:

  • In-state, fully valid driver's license or enhanced driver's license with photo (not temporary or learner's permit)
  • Official U.S. military or military dependent identification card
  • Government employee identification card (federal or state)
  • Current foreign passport

Secondary and supplemental identity documents, you must present two or more:

  • Out-of-state driver's license with photo
  • Learner's or temporary driver's permit
  • In-state, fully valid non-driver ID
  • Out-of-state, non-driver ID
  • Temporary driver's license
  • Expired driver's license
  • Student ID (College or High School)
  • Employee work ID
  • Social Security card

Both parents must appear and sign for a minor applicant and present current identification and proof of relationship to applicant (such as certified copy of birth certificate) when minor is under the age of 16.

Passport Fees

A U.S. Passport Book is the standard type of Passport. It can be used for all types of international travel, however if you intend to travel by air to an international destination, the passport book is the only document approved for international air travel.

U.S. Passport Cards are only good for crossing US borders by vehicle or as a government issued form of picture ID.

For each application processed by Utah County - TWO separate payments are required!

APPLICATION FEES MUST BE STAPLED TO YOUR APPLICATION

Application Fees are paid to the "US Department of State" and must be paid by a personal check, money order, or bank check. Credit/Debit cards or cash cannot be used to pay this fee!

Execution/Acceptance Fees are paid to the "Utah County Clerk" and can be paid using cash, check, or Visa, Mastercard, or Discover credit/debit card.

Passport photos are available at our office for $10.00 per person.

EXECUTION/ACCEPTANCE FEES ARE PAID SEPARATELY

Execution fees ($35) and photo service fees ($10) are paid directly to the Utah County Clerk. Our office accepts cash, checks, or Visa, MasterCard, or Discover debit/credit cards.

Passport photographs are available in the Clerk’s Office. Cost is $10.00 per person.
